Yesterday Miss Jaya Letchumi was admitted to M edical Ward 5 at HUKM. As a incharge nurse,its my responsibility to ill in the admission form for the new patient, Miss Jaya Letchumy. Here is what I have found after interviewing her.

From my observation, I come to know that Miss Jaya got a serious liver problem.It has been one week since she suffering from this condition. According to her the pain getting worse each day. It also attact more frequent lately.She said the pain feels like burning around her chest.Now, Miss Jaya says she starting having difficulties to breath.

But I believe Miss Jaya will be treat well here at HUKM. There is a lot of speacialist that will look after her. She will recover soon.

Noor Eziyan Syazwani is the Johor girl.She is eighteen years old.She life at Kulai,Johor.Her birthday on 15.8.1989 at Kudat,Sabah.Ncw she's studying for a diploma in nursing at National Univercity of Malaysia.She want to be a nurse because she loves to help people and likes working with people.She very interested in science because it's a challenging subject that need to study hard.

She'd like to be a scrub nurse because she want to supports surgeons in operating theatre.To her it's a challenging job and get a lot of experience.Now she's worked on surgery ward in HUKM as a student.She's hope that one day she will be a scrub nurse.

She's good at talking to people and make them feel comfortable and happy.She's a happy and a funny person that can make people laugh everyday.In her free time she like to goes out with her friends.She's also good in dancing because she always dance every Saturday.

My patient has severe abdominal pain.it began suddenly last 2days at 12pm as mild at beginning but getting worse at the end,this happens when he enjoying the party...it was constant pain all over the abdomen..my patient say that the pain is staying the same and is on the right-hand side to the abdomen and the pain also get worse when he coughs.

My patient(my partner) Ms.Fathin Nashuha bt Kamaruddin has liver pain.The pain suddenly began last night .The pain is mild and throbbing but its become worst this morning.Now the pain is moving around and she cant bare the pain.She dont know the course of the pain exactly.
